Because Afterbirth+ and its fifth booster pack both added achievements to the game but were released over a year apart, they are charted separately in these visualizations.

DLC Releases

This chart tracks the release dates for each DLC, and how long each had been available as of April 12, 2023 (when I pulled the achievement percentage data from Steam).

There's obviously a big difference between the first four releases and Repentance, which is also reflected in the spread of rare achievements.

Why are these the rarest?

The Real Platinum God

Unlocking 177 other achievements (a.k.a. secrets, which lead to endings), plus picking up ("collecting") 341 items is no mean feat, not least because some of those items have to be unlocked first, and unlocking items makes the overall item pool the game chooses from much larger, reducing your chances of encountering any specific item. With any DLC installed and enabled, the DLC items will further dilute your chances. However, Afterbirth makes some of the required achievements easier, namely the ones you can only get by playing as The Lost. The Lost has no health, so a single hit will kill him—but through an admittedly long and tedious process, Afterbirth allows you to unlock a shield for The Lost that refreshes every room, making this a somewhat easier achievement.

Mega

When this achievement was released, "every character" meant 13 characters. With Afterbirth+ enabled, 2 more characters are added, and with Repentance enabled, a further 2 are added. So if you have all DLC enabled, you have to beat Mega Satan with 17 characters. Like The Lost, some of these characters are incredibly challenging to play. And unlike other bosses, Mega Satan is behind a door that requires an item to unlock, besides being in chapter 6 (roughly the 10th floor). While arguably not the hardest boss in the game, of the major bosses that have achievements for each character, he has the lowest average defeat rate. Still, the item this achievement unlocks is incredibly powerful.

The chart at right shows the achievements each character gets for defeating major bosses up to Mega Satan. It includes the 13 characters in Afterbirth and the average defeat rate for each boss (the line). Later chapters take longer to reach, which means both more opportunity to gain items and more time to die before reaching the boss. One achievement for ??? was omitted because it can be unlocked by beating ??? as Judas or beating Ultra Pride (an uncommon miniboss that can occur on any floor) with any character, and therefore its extremely high 59.6% achievement rate does not reflect ??? defeats.

With Steam, you can purchase and install DLC without having it enabled, so it will not affect the game.

The Marathon

Daily Challenges or daily runs are random seeded runs generated daily. Any player can play them, and their score shows up on a global leaderboard for the day. But the only thing you can see before starting the run (see screenshot) is which character you'll be playing—not the items you start with, or the major boss you'll have to beat. They also cannot be played if mods are enabled, no matter what kind of mods you have. While you can skip daily runs and keep your win streak (e.g. to only play runs with characters you're good at), a five-win streak is still difficult.

Broken Modem

This achievement is easier than The Marathon because it requires seven wins total, not a streak. I suspect that it's slightly rarer because the achievement was not retroactive. Broken Modem the item was unlocked by default when Booster Pack 1 added it to the game, but Booster Pack 5 added the achievement to unlock it. When BP 5 locked Broken Modem, it didn't consider any daily run wins before BP 5, so you would have to get the 7 wins again to reunlock Broken Modem. Some players probably didn't care enough about the item to play more dailies, or didn't realize they were missing Broken Modem from their possible items.

Dead God

Not only has Repentance been out for only two years, but to get this achievement you have to complete 636 others AND pick up every item in the game at least once. It's literally the last achievement you can get. So it's not a surprise it's the rarest.
